Rebate Terms and Conditions:
The original rebate form must be fully completed with purchaser information filled out with valid and current data. Offer valid only in the US. Rebate paid in U.S. currency in the form of a check. All rebate requests must be postmarked by December 20, 2008 and received no later than December 31, 2008. Any requests postmarked or received after applicable dates will be ineligible. Receipt must include a date that falls within the mail-in rebate offer period. No reproductions of the receipt or rebate form will be accepted. Colavita USA, LLC is not responsible for late, lost, misdirected or postage due mail. This offer is only available to end-users. No rebate request from retailers or distributors will be honored. If terms and conditions are not met in Colavita’s sole discretion, rebate will not be sent. All decisions by Colavita are final.
