Purchase: Colavita Extra Virgin Olive Oil at 25.5 fl. oz. or larger. Offer good on product purchased between September 1, 2008 and August 28, 2009.
Rebate materials must be postmarked by August 28, 2009 and received by Colavita on or before August 31, 2009.

The original rebate form must be fully completed with purchaser information filled out with valid and current data. Offer valid only in the U.S. Rebate paid in U.S. currency in the form of a check. All rebate requests must be postmarked by August 28, 2009 and received no later than August 31, 2009. Any requests postmarked or received after applicable dates will be ineligible. Receipt must include a date that falls within the mail-in rebate offer period. No reproductions of the receipt or rebate form will be accepted. Colavita USA, LLC is not responsible for late, lost, misdirected or postage due mail. This offer is only available to end-users. No rebate request from retailers or distributors will be honored.
